My current role requires me to be able to work autonomously and as part of a Team due to the fact that I lone work on a number of my Shifts. I therefore feel confident in being able to manage challenging situations and assessing risk, knowing when to seek support. I also am competent in passing information over to Management and the rest of the Team through Handovers to ensure a smooth continuation of support. I ensure that information is recorded accurately throughout my Shift, documenting all support provided and any other issues that may occur. I am a Key Worker for an allocated number of Individuals within the Service and therefore am able to formulate Support Plans and Risk Management Plans with them in a person centred manner to ensure their aspirations and goals are being met through the Service. I regularly review these with the Individual making changes and documenting progress and outcomes. All Support Plans are SMART and goals are achievable.
I worked for the YMCA for two years with Individuals experiencing homelessness and my position was as a Housing Coach. During this time I completed my NVQ 3 in Supported Housing and NVQ level 2 in Youth Work. Throughout all of these roles, I have completed a number of other Courses which have equipped me with tools and knowledge to use such as Safeguarding Adults / Children, Self Harm Awareness, Suicide Intervention, First Aid at Work (3 Day), Recovery Coaching and Conflict Resolution.
